PHC supports practices applying for Equity and Practice Transformation Payments Program

Physicians for a Healthy California (PHC) is pleased to have offered application and technical assistance to independent practices during the first phase of the California Department of Health Care Services’ (DHCS) Equity and Practice Transformation Payments Program.
This one-time, $700 million primary care provider practice transformation program is intended to advance health equity and reduce COVID-19-driven care disparities by investing in upstream care models and partnerships to address health and wellness and fund practice transformation. These funds will pay for delivery system transformation payments to primary care practices (pediatrics, family practice, adult medicine primary care, primary care OB/GYN, and behavioral health providers in primary care settings) focused on advancing DHCS’ equity goals in the “50 by 2025: Bold Goals” initiative and to prepare them to participate in alternative payment models.
The work PHC has done so far with these practices included a comprehensive application review, providing edits and recommendations to ensure submission of competitive applications, and hosting office hours and weekly webinars that assisted practices with additional support.
During the second and third phases of this program, PHC will support participants in enacting their practice transformation goals, as well as ensuring appropriate monitoring and fund disbursement.
